Upon prior approval, we are glad to extend a payment plan of 6 months, no interest, through Care Credit or 12 months, no interest, through Citi Health Card. If you elect one of these services, the professional discount does not apply due to the administrative cost.

As a courtesy to you, we will file your dental insurance claim, provided you have given us the necessary information including the subscriber’s social security number. If you elect not to provide this information, we will ask for payment in full today. We will provide you with the appropriate insurance form so you can file your own dental claim. We require a payment of 30-50% of your treatment fee, depending on your insurance coverage. Your policy may base its allowances on a fixed fee schedule, which has been selected by your employer and your insurance company. These schedules and allowances may not coincide with our usual fees. Therefore, you will be solely responsible for any balance that is not paid by your insurance company for services rendered. For any account over 60 days, interest of 1.5% per month (18% APR) on the unpaid balance will accrue on your account until it is paid in full.

Prior to treatment, you will be informed of fees for any other services provided by our office such as endodontic surgery or retreatment.
