In order for us to keep our fees as low as possible, we would like to minimize monthly billing by offering a professional courtesy of 5% of
the endodontic treatment fee for payment in full the day of your visit. Should your treatment require a second appointment, a minimum of 50% of the treatment fee is due
on your first visit and the balance at
completion. Should your appointment be for a consultation, payment is due in full. We accept Visa, Mastercard, Discover, American Express, Cash and Personal Checks. As a courtesy to you, we will file your
dental insurance if you will provide us with all the necessary information. Because we do not know exactly what your insurance will cover, we require a
payment of 20-50% depending upon your insurance
coverage for each endodontic procedure. You should
be aware that insurance companies vary greatly in the
types of coverage. Your policy may base its
allowances on a fixed fee schedule which has been
selected by your employer and your insurance company.
These schedules and allowances may not coincide with our
usual fees. If there is an additional amount due,
you will be responsible and a statement will be sent to
you. For any account over 60 days, interest of 1.5% per month (18% APR) on the unpaid balance will accrue in your account until paid in full.
As
a service to our patients, we are pleased to offer the CareCredit card, which
offers a six (6) month, no interest plan for financing.
